For new patients interested in our services, HarmonyCares will send a healthcare provider to the patient’s home for an initial visit. After filling out the form below, a HarmonyCares representative will contact you by phone or email within 1-2 business days.
If you are ready to register yourself or a patient for our services and have traditional Medicare, you can complete the registration below.
To complete registration you will need the following:
- Traditional Medicare card
- Secondary insurance card (if applicable)
If you need immediate assistance regarding a current HarmonyCares patient, please contact the patient’s HarmonyCares office.
